I appreciate the effort you put in to protect your Forum.
This issue basically boils down to that somehow, these affiliates get information from people either by them signing up on Affiliate/Surveys/Dodgy Casinos or similar sites and then the Phone Number is "Out there". You can rest assured that any details provided to Frank & Fred will never be shared with a 3rd party. But once it's "Out there", these lists will be circulated by being traded or sold to other similar partners. And since it's in the individual Affiliate's interest to get "as much traffic as possible" these lists will be used for years to drive traffic to different Casinos, earning the Affiliate in question money. It's all down to money in the end. As long as it works, they will keep doing it.
The problem for us is that we have 100's of partners sending us traffic, and we get more all the time. When they register with us, our Affiliate team does a due diligence, ensuring the Partners site and/or database is "clean" and up to our standards. But there is only so much we check, and it's only so much data the partner will provide us with. And in the end, we do not control the partner's Website, Database and/or Ads. They run their business and we run ours.
So, to answer your question, No, I am not certain it's fixed and that this will never happen again. We can only take responsibility for that any partner found in breach of the terms will be dealt with.
A general rule of thumb is to never sign up with personal details to anything that looks dodgy. Even if there's a chance to win an iPhone or such. Very few businesses on the world wide web are not looking to make money. They will earn their money somehow, and this includes spamming Casino links to a database of phone numbers.
